Victim Impact Panel - Maricopa County

Telephone Registration Instructions:   

  • You must call 602-240-6500, press 2 (English) or 7 (Spanish) - then listen carefully to the detailed class instructions.
  • To register you must SPEAK CLEARLY and leave the following info:
  •          1) The DATE and LOCATION of the class you wish to attend (ie) the February 4th class in MESA.
  •          2) The spelling of your FIRST and LAST NAME - as it appears on your Court paperwork
  •          3) Your CASE NUMBER   (sometimes referred to as a Docket Number.  If you do not see a mention of a Case or Docket Number on your paperwork, mention this on your voicemail.  If you do not have this information at time of your telephone registration, make sure you bring it with you to the class)
  •          4) The name of the COURT or AUTHORITY that ordered you to attend this class;
  •          5) A contact TELEPHONE NUMBER where you can be reached should we have any questions, or provide details of any class changes.
  •          Remember, if you do not leave a working contact telephone number, we have no way of contacting you.
  • Due to the high volume of calls we receive, it would be impossible to call people to confirm registration. If you have used the automated system, rest assured that you are registered successfully. A return call will be made ONLY if there is a question regarding your registration or scheduling changes.
  • If for any reason you cannot attend your scheduled class you must contact our office by 1 p.m of the day of your class, otherwise you will be subject to a $10.00 rescheduling fee. (see Class Fees section below for further details)

Class Instructions:   

  • It is recommended attendees arrive between 6.00 pm and 6.30 pm to sign-in and pay their class fee.
  • Class begins at 6.45 pm sharp , and will end at approximately 8.45 pm.
  • Late arrivals will be turned away and will have to reschedule for a future class.
  • You need to bring THREE things with you to the class:
    • 1) A PICTURE ID
    • 2) Your CASE NUMBER  (sometimes referred to as a Docket Number.  We recommend that you bring your case paperwork with you to the class) 
    • 3) Your CLASS FEE - payable by Money Order only (see Class Fees section below for full details on how to complete your money order)
  • Alcohol consumption on the day of the panel is not permitted. 
  • A breathalyzer test must be passed to enter the panel. 
  • Late arrivals will be turned away and you will have to reschedule.
  • At the end of the presentation you will receive a certificate that you will need to bring to the sentencing court for proof of compliance.  You are also responsible for keeping a copy for you own personal records.
  • Attendees are authorized to bring guests with them to the class, at no extra charge.  Guests will not receive a certificate.
  • Due to the graphic nature of the program, children under the age of 14 will not be admitted.  We reserve the right to request proof of age. 

Class Fees:    

  • Class Fee for Maricopa County classes is $40.00 , as of 1/1/2008. 
  • We accept Money Order or Cashiers Check ONLY .   No Cash or Personal Checks!
  • If you arrive at the class without a Money Order / Cashiers Check, you will have to reschedule for a future class. 
  • Please Note!  Your completed Money Order / Cashiers Check needs to contain TWO important pieces of information:
    • 1)  Your Money Order / Cashiers Check is to be made payable to MADD.
    • 2)  Your FIRST and LAST NAME must be written clearly on the Money Order, in an easy to read fashion. 
    • Upper Case / Block Capital Letters are desired.  Scrawled or illegible signatures are not acceptable. 
    • Presenting an incomplete or hard to read Money Order will result in you losing your place in line - you would then have to rejoin the back of the line and requeue once you had completed the desired information.
  • $10.00 Rescheduling Fee:  
    • If you fail to notify our office of your need to reschedule or cancel a class by 1pm on the day of the class, a $10.00 Rescheduling Fee shall apply.  The $10.00 Rescheduling Fee shall apply to each and every instance you fail to attend or notify us on time. 
    • Miss one class, your fee is $50.00.  Miss two, your fee is $60.00, and so on. 
    • Each time you fail to attend, we have to re-register you, and generate a new certificate - hence the Rescheduling Fee.
  • $10.00 Replacement Certificate Fee: 
    • We provide you with the original copy of your Certificate upon completion of the class. 
    • We do not keep a copy, therefore, if you lose or misplace your certificate for whatever reason, and need a copy, there is a $10.00 fee to generate a new certificate. 
    • To do so, we need to know your name, your case number, and the date and location you attended the class.

MESA:     

  • Location:            City Of Mesa Utilities Department, in the Community Room building.  The Community Room building is located to the immediate left of Reception.
  • Address:             640 N. Mesa Drive, Mesa, AZ 85201 
  • Additional Info:     640 N Mesa Drive is located a few blocks south of University Drive, between E. 6th Street and E. 8th Street.

Mesa Classes are held on a Monday, unless indicated otherwise.

  • Location:             Scottsdale United Methodist Church, in the Burgess Hall.
  • Address:             4140 N. Miller Road, Scottsdale, AZ 85251 (located between Indian School Road and Camelback Road)
  • Additional Info:     The Church is approximately 100 ft north of Indian School, on the west side of the street, behind the CVS Pharmacy.

Scottsdale Classes are held on a Monday.

  • Location:             Good Samaritan Hospital, in the Amphitheater.
  • Address:             1111 E. McDowell Road, Phoenix, AZ 85006 (located between 11th and 12th Streets)
  • Additional Info:     The Ampitheater is located in Lower Level 2 of the hospital.
  • Additional Info:     Therefore, go to the main Hospital Reception Desk / Lobby and follow the MADD signs which lead to the class.
  • Additional Info:     If you arrive early, and there are no signs posted yet, the Receptionist Desk can provide directions for you.

Central Phoenix Classes are held on a Wednesday.

GLENDALE:   

  • Location:             Independence High School, in the Mini Auditorium.
  • Address:             6602 N. 75th Avenue, Glendale, AZ 85303 (located on the corner of 75th Avenue and Maryland Avenue)
  • Additional Info:     Maryland Avenue is approximately halfway between Glendale Road and Bethany Home Road. 
  • Additional Info:     Follow the signs and park in the Student Parking Lot.  Signs are also posted leading to the class.

Glendale Classes are held on a Thursday.

  • Location:             South Mountain Community College, in the Student Union.
  • Address:             7050 S. 24th Street, Phoenix, AZ 85042 (located between Vineyard Road and Baseline Road)
  • Additional Info:     The Student Union is located across from the Cafeteria.
  • Additional Info:     South Phoenix is our only bilingual panel. 
  • Additional Info:     Both the English and Spanish language classes are run at the same time, in seperate classrooms.

South Phoenix Classes are held on a Tuesday, unless specified otherwise.
