Victim Impact Panel - All Other Counties

Telephone Registration Instructions for Mohave, Navajo and Yavapai Counties:

(For Cochise, Coconino, Pima, Pinal and Santa Cruz - scroll down to the bottom of the page)   

  • You must call 602-240-6500, press 2 (English) or 7 (Spanish) - then listen carefully to the detailed class instructions.
  • To register you must SPEAK CLEARLY and leave the following info: 
    • 1) The spelling of your FIRST and LAST NAME - as it appears on your Court paperwork;
    • 2) Your CASE NUMBER  (sometimes referred to as a Docket Number.  If you do not see a mention of a Case or Docket Number on your paperwork, mention this on your voicemail.  If you do not have this information at time of your telephone registration, make sure you bring it with you to the class)
    • 3) The name of the COURT or AUTHORITY that ordered you to attend this class; 
    • 4) A contact TELEPHONE NUMBER where you can be reached should we have any questions, or provide details of any class changes. 
    • Remember, if you do not leave a working contact telephone number, we have no way of contacting you.
  • Due to the high volume of calls we receive, it would be impossible to call people to confirm registration. If you have used the automated system, rest assured that you are registered successfully. 
  • A return call will be made ONLY if there is a question regarding your registration or scheduling changes.
  • If for any reason you cannot attend your scheduled class you must contact our office by 1 p.m of the day of your class, otherwise you will be subject to a $10.00 rescheduling fee. (see Class Fees section below for further details)

Class Instructions:   

  • Class sign-in for all non-Maricopa County classes begin at different times.  See the 'Additional Info' section of your County for further details. 
  • Late arrivals will be turned away and will have to reschedule for a future class.
  • You need to bring THREE things with you to the class:
    • 1) A PICTURE ID
    • 2) Your CASE NUMBER  (sometimes referred to as a Docket Number.  We recommend that you bring your case paperwork with you to the class) 
    • 3) A MONEY ORDER or CASHIERS CHECK for the appropriate amount (see Class Fees section below for full details on how to complete your money order / cashiers check)
  • Alcohol consumption on the day of the panel is not permitted. 
  • A breathalyzer test must be passed to enter the panel. 
  • Late arrivals will be turned away and you will have to reschedule.
  • At the end of the presentation you will receive a certificate that you will need to bring to the sentencing court for proof of compliance.  You are also responsible for keeping a copy for you own personal records.
  • Attendees are authorized to bring guests with them to the class, at no extra charge.  Guests will not receive a certificate.

Class Fees:    

  • See the 'Additional Info' section of your County for the applicable Class Fee. 
  • We accept Money Order or Casheirs Check ONLY.   No Cash or Personal Checks!
  • If you arrive at the class without a money order or cashiers check, you will have reschedule for a future class. 
  • Please Note!  Your completed Money Order / Cashiers Check needs to contain TWO important pieces of information:
    • 1)  Your Money Order / Cashiers Check is to be made payable to MADD.
    • 2)  Your FIRST and LAST NAME must be written clearly on the Money Order, in an easy to read fashion. 
    • Upper Case / Block Capital Letters are desired.  Scrawled or illegible signatures are not acceptable. 
    • Presenting an incomplete or hard to read Money Order will result in you losing your place in line - you would then have to rejoin the back of the line and requeue once you had completed the desired information.
  • $10.00 Replacement Certificate Fee for lost/misplaced Certificates:
    • We provide you with the original copy of your Certificate upon completion of the class. 
    • We do not keep a copy, therefore, if you lose or misplace your certificate for whatever reason, and need a copy, there is a $10.00 fee to generate a new certificate. 
    • To do so, we need to know your name, your case number, and the date and location you attended the class.

MOHAVE COUNTY    

  • Location:             Mohave County Sheriffs Office.
  • Address:             600 W. Beale Street, Kingman, AZ 86401
  • Additional Info:     DO NOT CALL THE SHERIFF'S OFFICE TO REGISTER
  • Additional Info:     Mohave County Classes occur every other month.
  • Additional Info:     Class fee for Mohave County classes is $30.00.  Money Order / Cashiers Check only.  NO CASH.
  • Additional Info:     Class sign-in begins at 6.30 pm
  • Additional Info:     Class begins at 7.00 pm sharp and ends at approximately 8.30 pm.

Mohave County Classes are held on a Tuesday.

NAVAJO COUNTY

  • Additional Info:     Navajo County Classes occur every other month.
  • Additional Info:     Class fee for Navajo County classes is $30.00.  Money Order / Cashiers Check only.  NO CASH.
  • Additional Info:     Class sign-in begins at 6.30 pm
  • Additional Info:     Class begins at 7.00 pm sharp and ends at approximately 8.00 pm.

Navajo County Classes are held on a Wednesday, unless specified otherwise.

  • Additional Info:     As per their request, the Court or Authority that is sending you to the class shall handle your registration, not MADD.
  •                           If you are unable to attend your assigned class, you should contact the Court or Authority as soon as possible to
  •                           reschedule.  MADD cannot schedule or reschedule your class.
  • Additional Info:     Do not contact The Old Fairgrounds for class information / class rescheduling as they would not be able to assist you.

  • Additional Info:     As per their request, the Court or Authority that is sending you to the class shall handle your registration, not MADD.
  •                           If you are unable to attend your assigned class, you should contact the Court or Authority as soon as possible to
  •                           reschedule.  MADD cannot schedule or reschedule your class.
  • Additional Info:     Do not contact The Prescott Police Dept for class information / class rescheduling as they would not be able to assist you.

  • Additional Info:     Prescott Juvenile Classes occur every other month.

  • Additional Info:     Do not contact Cottonwood Police Dept for class information / class rescheduling as they would not be able to assist you.

Registration Instructions for Cochise, Coconino, Pima, Pinal and Santa Cruz Counties:

 

COCHISE, PIMA, PINAL AND SANTA CRUZ COUNTIES    

  • Our Southern Arizona Affiliate Office in Tucson runs the classes in Cochise, Pima, Pinal and Santa Cruz Counties. 
  • Call MADD Southern Arizona on 1-877-255-6233 to register for a class in Cochise, Pima, Pinal or Santa Cruz County.
  • Please visit http://www.madd.org/az/pima and click on 'Victim Impact Panel Schedule' to view class schedules for Cochise, Pima, Pinal and Santa Cruz Counties.

 

COCONINO COUNTY    

This class is facilitated by Victim Witness Services For Coconino County, Coconino County Adult Probation Department and MADD.  Telephone registration is no longer required for the class held in Coconino County.  To register and attend, simply show up to class.  Seating is unlimited, therefore you need not worry about the class being full. 

  • Location:             Flagstaff Medical Center, in the McGee Auditorium.
  • Address:             1200 N. Beaver Street, Flagstaff, AZ 86001
  • Additional Info:     The McGee Auditorium is located on the first floor, in the Women and Infant's Center.
  • Additional Info:     Class fee for Coconino County classes is $20.00 - payable by cash, money order or cashiers check
  • Additonal Info:      Coconino class is the only class where cash is accepted.
  • Additional Info:     Class sign-in / registration begins at 5.40 pm.
  • Additional Info:     Class begins at 6.00 pm sharp and ends at approximately 7.30 pm.
  • Additional Info:     Photo ID is required.
  • Additional Info:     Attendees will need to pass a breathalyzer before being admitted to class.
  • Additional Info:     You will receive a certificate of completion at the conclusion of the class.
  • Additional Info:     DO NOT CALL THE MADD OFFICE TO REGISTER.  Simply show up to the Flagstaff Medical Center as per the instructions above.

Coconino County Classes are held on a Wednesday.
