Victim photo release/consent form

MADD VA welcomes submissions of pictures of drunk driving crash victims for inclusion on one of our statewide Victim Boards. Just send us a picture, along with this consent form, completed. We will add the victim’s name, the date of the crash, and a few words about the details of the crash and/or the victim, before laminating the photo.
